I understand that martial arts, combat sports and fitness training involve physical contact and carry risks including injury, disability or death.

I voluntarily assume all risks associated with training, sparring, fitness activities, competitions and participation in academy programs.

I understand that risks may arise from my own actions, the actions of others, training activities, premises conditions or equipment used.

I certify that I am physically fit and do not suffer from any condition that would make participation unsafe.

I authorize emergency medical treatment if required due to injury or medical emergency during participation.

I agree to follow all rules, regulations, instructions and safety guidelines provided by Bodhiraan Judo & MMA Academy and its instructors.

I understand that fees paid are non-refundable and failure to attend classes does not relieve payment obligations.

I agree to use martial arts skills responsibly and only for lawful and self-defense purposes.

I release and discharge Bodhiraan Judo & MMA Academy, its owners, coaches, instructors, employees and representatives from liability arising from participation except where prohibited by law.
